Toronto General & Western Hospital Foundation is establishing a Transition Navigation System to support to support young adults with Sickle Cell Disease. In addition to the normal challenges of growing up, young adults with Sickle Cell Disease have the added burden of managing debilitating physical ailments. Compounding these challenges, Sickle Cell Disease primarily affects a marginalized population. Among its activities, the Foundation’s Red Blood Cell Disorders program, which provides youth with a direct link between paediatric and adult care, is developing a tool box of educational resources for the successful care of the patient group and a resource of best practices available to other similar programs.
